Privacy Notice: The free service is designed to support patients at risk of osteoporosis (the “Program”) and is provided by IQVIA RDS, East Point Business Park Estuary House, Fairview, Co. Dublin, D03 Y6A2, Ireland (“IQVIA”) on behalf of Amgen Ireland Ltd, 21 Northwood Court, Santry, Dublin 9 (“Amgen”), the Data Controller.
The Data Processor, IQVIA is the organization that collects your personal information, on behalf of Amgen. The information you provide will be used by IQVIA to contact you to provide you with information about the program and give you the option to make an appointment with the Osteoporosis Specialist Nurse (“Purpose”). Your personal information will be processed by IQVIA only for the Purpose and in the manner described above. IQVIA maintains appropriate technical and organizational measures to protect your personal information from unauthorized or unlawful access, accidental loss, alteration, or destruction, in full compliance with local data protection regulations.
Should you elect to participate in the Program, IQVIA will provide you with further information and you will be asked to complete and sign a consent form which will, amongst other information, also set out how your personal data will be used for the purpose of the Program.
Should you, after contact by IQVIA, elect not to participate in the Program, your personal information will be securely destroyed. In some instances, IQVIA and/or Amgen may be required by the applicable law to retain your personal information for a period highlighted by the corresponding law. If IQVIA is unable to contact you, your details will be held for a maximum of 30 days.
